How we may use and disclose your medical information.

The Practice may use and disclose your information for purposes related to your care, payment for your care and operations of the Practice.  The following are examples of such uses and disclosures, and are not meant to possible types of uses and disclosures.

• For Care – The Practice will provide your health records too health care professionals that are directly involved in your care so that they may understand your medical condition and needs, advice or treatment to you. 
• For Payment – In order to get paid for the health care provided by the Practice, the Practice will send your records to the appropriate third party payors, such as insurance carrier.
• For Health Care Operations – In order for the Practice to provide quality and efficient care to you. It is necessary for us to compile, use and disclose your health records for health care operations purposes.
